6022.2025 Development of Group Antenatal Care Package in the Desa SIAGA Context: Qualitative Focus Group Design in collaboration with Stakeholders

Abstract

The urgency of this study is grounded in the persistently high burden of maternal and

neonatal health (MNH) in Indonesia. Maternal mortality declined from 474 (2000) to 140 per

100,000 live births (2023) yet remains above the WHO target of 70 and the regional

average. Neonatal mortality fell from 22.8 to 11 per 1,000 live births, but Indonesia still

ranks high in Southeast Asia. Key barriers include uneven quality of maternal services,

limited provider competence, and gaps in essential services. The Desa SIAGA programme

seeks to strengthen birth preparedness; one of its components, the pregnancy class (kelas

ibu hamil, KIH), can increase ANC use and facility delivery but is typically didactic, not

integrated with clinical assessment, and faces implementation constraints. These

limitations highlight the need for approaches that are interactive, integrated, and woman

centred. To address this gap, Group Antenatal Care (Group ANC)—combining clinical

assessment, participatory learning, and peer support—has shown positive results

internationally, and Phase One of this research indicated its acceptability in the Desa

SIAGA context. This Phase Two study aims to develop a culturally contextualised, Desa

SIAGA–integrated prototype Group ANC package through Focus Group Discussions

(FGDs). Methods: Two FGDs (eight participants each) will be conducted using purposive

sampling in Ngerangan and Kujon (Klaten). Group 1 will include pregnant and postpartum

women; Group 2 will comprise village midwives, the village head, cadres, district health

office staff, the head of the community health centre, and an academic. Groups are

separated to minimise power dynamics. Demographic data will be analysed descriptively;

qualitative data will be analysed using Reflexive Thematic Analysis to generate context-

sensitive, practice-relevant themes that guide design and implementation. Outputs include

a prototype Group ANC package and a manuscript for submission to a Scopus-indexed

international journal - Jurnal Ners (https://e-journal.unair.ac.id/JNERS). Technology
