Abstract

Background:
Falls constitute a major cause of injury-related morbidity and mortality among older adults worldwide. In Indonesia, approximately 29% of community-dwelling older individuals have experienced at least one fall, underscoring the urgency of effective preventive strategies. Exercise interventions, particularly those emphasizing strength and balance, have consistently demonstrated efficacy in reducing fall risk. However, limited accessibility remains a significant barrier. While hybrid (online and offline) exercise programs have shown potential in improving outcomes among older adults in high-income settings, their implementation and evaluation in the Indonesian context remain limited.

Objective:
This study aims to design and assess a hybrid exercise program for fall prevention among Indonesian older adults by evaluating readiness, feasibility, and effectiveness across different settings.

Methods:
A mixed-methods approach will be adopted, comprising  sequential phases. The first phase involves a systematic review of digital and hybrid exercise interventions targeting fall and frailty management in Southeast Asia. The second phase entails a cross-sectional survey conducted in urban and rural areas of Sukoharjo, Central Java, to assess older adults’ digital readiness, including their familiarity with technology and willingness to engage in hybrid exercise programs. In the final phase, a 12-week randomized controlled trial will be conducted in an urban setting to examine the feasibility and effectiveness of the developed intervention. Functional and cognitive measures will be assessed pre- and post-intervention to determine outcomes.
